IMPORTANT NOTICE
Early Registeration is scheduled on Monday, June 29th and Tuesday, June 30th, 2009Â between the hours of 9:00am and 12:00pm
TO REGISTER A STUDENT IN A NEW YORK CITY PUBLIC SCHOOL
- Parent/Guardian must be present
- Student must be present
ALL DOCUMENTATION REQUIRED:
- Proof of age (birth certificate, baptismal certificate or passport)
- Proof of immunization
- Recent report card or transcript
- Proof of address
ACCEPTABLE DOCUMENTS ACCORDING TO CHANCELLORS REGULATIONS:
- Residential utility bill (gas or electric) in the resident’s name within the prior 60 days
- Documentation or letter on letterhead from a federal, state, or local government agency, including the IRS, City Housing Authority, Human Resources Administration, Administration for Children’s Services (ACS) Original lease agreement, deed or mortgage statement for the residence
- Property tax bill for the residence
- Water bill for the residence
- Official payroll documentation from an employer issued within the past 60 days such as a form submitted for tax withholding purposes or a payroll receipt.
- None of the documents will suffice alone. Two of the above are required.
UNACCEPTABLE DOCUMENTS ACCORDING TO CHANCELLORS REGULATIONS:
- Telephone bill
- Cable bill
- Credit card bill
- Medical insurance card
- Driver’s license
- A letter on the employer’s letterhead
