IMPORTANT NOTICE

Early Registeration is scheduled on Monday, June 29th and Tuesday, June 30th, 2009 between the hours of 9:00am and 12:00pm

TO REGISTER A STUDENT IN A NEW YORK CITY PUBLIC SCHOOL

• Parent/Guardian must be present
• Student must be present

ALL DOCUMENTATION REQUIRED:

• Proof of age (birth certificate, baptismal certificate or passport)
• Proof of immunization
• Recent report card or transcript
• Proof of address

ACCEPTABLE DOCUMENTS ACCORDING TO CHANCELLORS REGULATIONS:

• Residential utility bill (gas or electric) in the resident’s name within the prior 60 days
• Documentation or letter on letterhead from a federal, state, or local government agency, including the IRS, City Housing Authority, Human Resources Administration, Administration for Children’s Services (ACS) Original lease agreement, deed or mortgage statement for the residence
• Property tax bill for the residence
• Water bill for the residence
• Official payroll documentation from an employer issued within the past 60 days such as a form submitted for tax withholding purposes or a payroll receipt.
• None of the documents will suffice alone. Two of the above are required.

UNACCEPTABLE DOCUMENTS ACCORDING TO CHANCELLORS REGULATIONS:

• Telephone bill
• Cable bill
• Credit card bill
• Medical insurance card
• Driver’s license
• A letter on the employer’s letterhead
